The Minhag of Tashlich is from the Maharil.
After Mincho of first day Rosh Hashono, everyone goes to the river or whatever they have similar to that, and says:
מי קל כמוך נשא עוון ועובר על פשע לשארית נחלתו, לא החזיק לעד אפו כי חפץ חסד הוא
ישוב ירחמנו יכבוש עוונותינו, ותשליך במצולות ים כל חטאותם
וכל חטאת עמך בית ישראל תשליך במקום אשר לא יזכרו ולא יפקדו ולא יעלו על לב לעולם
תתן אמת ליעקב, חסד לאברהם אשר נשבעת לאבותינו מימי קדם
After that, one shakes the edges of his jacket or his pockets, to show that we would like to throw our sins away. Nothing beside the sins is thrown into the water. If the first day Rosh Hashono is on Shabbos - this is done in the second day.
A Minyan is not mentioned, but since everyone goes together after Mincho there is usually a very big Minyan.
