Leining the haftoroh in absence of sefer aftartoh was created by Alexander Weil
If one cannot obtain a sefer aftartah, is it preferable for the ba'al maftir to lein out of a tanach (like is done in many shuls in America) or out of the back of a chumash where each week's haftarah is printed separately?

If you don't have a Seifer Aftarto, the most important thing is to try to find a book printed in old print (as it was 30 years ago), when the letters were physically printed into the pages (similar to the typewriter of those days) and not only copied (photo offset) to the pages as today.
If you have two books in old print, one a Tanach and one a Chumash, you can do the Chumroh of the Mogen Avrohom to use a Tanach (not to rely on the Eis Laasos LaHashem), but that is not Minhag Ashkenaz. According to Minhag Ashkenaz one can lein the Haftoro out of a Sefer Aftarto, even if he has a full Novi available.
The Chasam Sofer says, that Bedi'eved, one can say the Haftoro by heart also Me'ikar Hadin.
