וְעַל נִשְׁמוֹתֵינוּ הַפְּקוּדוֹת לָךְ
or
וְעַל נִשְׁמוֹתֵינוּ הַמוּפְקָדוֹת לָךְ

The question is, what does the first mean? The translations given in various places (Rav Hirsch, Artscroll etc.) seem to translate the second phrase.
